General Consent Statement:It is agreed that the above named child may take part in football training, matches, tournaments, and events organised by the club. It is understood that while involved, my child will be under the control and care of the team coaches and/or other adults approved by the club committee. Furthermore, it is understood that, while the staff in charge of any club activities that my child may participate in, will take all reasonable care of the children, they cannot necessarily be held responsible for any loss, damage, or injury suffered during, or as a result of, these activities. Photography & Filming statement:It is understood that photography and/or filming may take place at club activities for media coverage and/or publications to advertise club activities. Any issues with photography and/or filming at club activities must be addressed with the club in advance of completing this Membership Registration Form via emailing secretary@southashfordfc.co.uk (See Club Policies for more details)
Player Fines Statement:Parents/Guardians will be responsible for payment to the club of any fines or administration costs incurred by their child whilst playing for South Ashford FC. These will normally be through a sending off or caution. (See Club Policies for more details)
Transport Statement:It is acknowledged that any arrangement to transport my child to or from club activities is the responsibility of parents/guardians. Determining the insurance or roadworthiness of vehicles or drivers is not the responsibility of South Ashford FC. (See Club Policies for more details) Kit & Equipment:All Kit and equipment provided by South Ashford FC remain the property of South Ashford FC, and must be returned to the football club immediately upon request or as and when a player leaves the Club, unless purchased by a parent/guardian or gifted by the club.

Membership Registration is not complete until this form has been Signed and Submitted, the required Membership Registration Fee has been paid and a clear head/shoulders style photo of the above-named child is received by the club.
Membership Registration fee is £45.00* to be paid as a one-off instalment during the registration process.

Membership Registration fee is non-refundable.
Player Subscription fee instalments should be made regardless of matches played, or training sessions attended, non-payment by the end of each month may see player membership suspended (training and match day selection) until the issue is resolved.
The Membership Registration and Yearly Player Subscription fees contribute to the following: FA affiliation fees, Local football league entry fees, Insurances, Player entry fees into local leagues, Training facilities, Match facilities, Training and match equipment, Tournament entry fees, End of season awards.
